Acceptance of this Research Use Only (RUO) Acknowledgement is a mandatory condition of purchase.

By selecting the Research Use Only acknowledgement checkbox during checkout, account registration, order submission, or any other electronic acceptance process provided by Reviva, you acknowledge that you have read, understood, and agreed to all statements contained in this Research Use Only Acknowledgement.

If you do not agree, you must not purchase Products from Reviva.

Your electronic acceptance shall have the same legal effect as a handwritten signature.

Acceptance records, including timestamps, IP addresses, account information, checkout records, electronic signatures, order records, and related electronic records, may be retained by Reviva for compliance, legal, fraud prevention, security, and record-keeping purposes.

By accepting this Research Use Only Acknowledgement, I represent, warrant, acknowledge, and agree that:

• I am at least twenty-one (21) years of age, or the age of majority in my jurisdiction, whichever is greater.
• I have the legal capacity to enter into binding agreements.
• I am legally permitted to purchase, possess, import, and use the Products in my jurisdiction.

I have read, understood, and agree to be bound by Reviva’s:

• Research Use Disclaimer
• Shipping Policy
• Returns, Refunds and Cancellation Policy
• Privacy Policy
• Platform Use & Acceptable Use Policy
• Age Verification & Eligibility Policy

as amended from time to time.

I understand and acknowledge that all Products sold by Reviva are intended strictly for:

• Laboratory use
• Analytical use
• Educational use
• Research use

and for no other purpose.

I will not:

• Consume any Product.
• Ingest any Product.
• Inject any Product.
• Inhale any Product.
• Apply any Product to a human or animal body.
• Administer any Product to a human or animal.
• Use any Product for therapeutic purposes.
• Use any Product for diagnostic purposes.
• Use any Product for veterinary purposes.
• Market any Product for human or veterinary use.

I understand and acknowledge that Products sold by Reviva:

• Are not approved pharmaceuticals.
• Are not dietary supplements.
• Are not foods.
• Are not cosmetics.
• Are not medical devices.
• Are not veterinary medicines.

I further understand that Products have not been evaluated, approved, authorized, licensed, cleared, registered, or certified for human or veterinary use by:

• United States Food and Drug Administration (FDA)
• UAE Ministry of Health and Prevention (MOHAP)
• Dubai Health Authority (DHA)
• Department of Health – Abu Dhabi (DOH)
• Gulf Health Council (GHC)
• Saudi Food and Drug Authority (SFDA)
• Bahrain National Health Regulatory Authority (NHRA)
• Qatar Ministry of Public Health (MOPH)
• Kuwait Ministry of Health
• Oman Ministry of Health

or any comparable regulatory authority unless expressly stated otherwise.

I represent and warrant that my purchase, possession, importation, transportation, storage, research use, and handling of Products complies with all applicable laws, regulations, and requirements in my jurisdiction.

I accept sole responsibility for determining the legality of Products in my jurisdiction.

I acknowledge that I am solely responsible for the handling, storage, transportation, possession, research use, and disposal of Products after delivery.

I voluntarily assume all risks associated with the purchase, possession, transportation, storage, handling, and use of Products.

To the fullest extent permitted by law, I release, indemnify, defend, and hold harmless Reviva LLC and its owners, directors, officers, employees, contractors, suppliers, affiliates, successors, and representatives from and against any claims, liabilities, losses, damages, costs, expenses, penalties, or legal fees arising out of:

• My purchase of Products.
• My possession of Products.
• My transportation of Products.
• My storage of Products.
• My handling of Products.
• My misuse of Products.
• My violation of applicable laws.
• My violation of Reviva policies.

I agree that electronic acceptance of this Research Use Only Acknowledgement shall have the same force and effect as a handwritten signature.

I further acknowledge that electronic records maintained by Reviva may be used as evidence of my acceptance.

This Acknowledgement shall be governed by the laws of the United Arab Emirates. Any dispute shall be subject to the exclusive jurisdiction of the courts of the United Arab Emirates.
